The ANNUAL Release Forms can be printed here in advance of arrival or sign the hard copy poolside. Annual waivers are required to participate and are due on day one of lessons for all swimmers. All Seahorse Swimmers at the Santa Cruz Elks Lodge agree to the Rules – see rules laminated on each table poolside. Please review & agree, sign the annual waiver documents to ensure all enrolled students can participate.
